Lipid Profile (Cholesterol) Test: How to Read Your Results
A lipid profile test comes back with four numbers. Most people look at the first one, total cholesterol, and stop there. But that single number tells you only part of the story. LDL, HDL, and triglycerides each measure a different aspect of how fat moves through your bloodstream, and together they give a much clearer picture of your cardiovascular risk than total cholesterol alone.
This guide walks through each number in turn, explains what the normal ranges mean, and tells you what to do if a result falls outside them.
What the lipid profile test measures
A lipid profile is a panel of four blood measurements that together map your cardiovascular risk: total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. Some labs also calculate the total-to-HDL ratio automatically, which cardiologists use as a quick risk index.
Cholesterol is a waxy substance your body makes and also gets from food. It travels through the bloodstream packaged in proteins called lipoproteins. LDL and HDL are the two main lipoprotein types; they carry cholesterol in opposite directions and have opposite effects on artery health. Triglycerides are a separate class of fat that the body stores as energy and releases between meals.
Most Ghanaian labs report results in millimoles per litre (mmol/L). Some international reports use milligrams per decilitre (mg/dL). Both measure the same thing at different scales. To convert cholesterol values from mmol/L to mg/dL, multiply by 38.7. Triglycerides use a different factor: multiply by 88.6.
Total cholesterol: what the range means
Total cholesterol below 5.2 mmol/L (200 mg/dL) is considered desirable by the NCEP ATP III guidelines and the ACC/AHA. This number is the sum of all the cholesterol-carrying particles in your blood, including LDL, HDL, and a smaller fraction called VLDL.
- Desirable: below 5.2 mmol/L (below 200 mg/dL)
- Borderline high: 5.2 to 6.2 mmol/L (200 to 239 mg/dL)
- High: 6.2 mmol/L (240 mg/dL) or above
One caution with total cholesterol: it is possible to have a total reading in the desirable range while still carrying a high cardiovascular risk, if your LDL is elevated and your HDL is low. This is why labs report all four numbers rather than total cholesterol alone.
LDL cholesterol: the number your doctor watches most
LDL cholesterol below 2.6 mmol/L (100 mg/dL) is the optimal target for most healthy adults, according to the 2018 ACC/AHA cholesterol guidelines. LDL is often called 'bad' cholesterol because it deposits in artery walls and contributes to the plaques that narrow blood vessels over time.
- Optimal: below 2.6 mmol/L (below 100 mg/dL)
- Near optimal: 2.6 to 3.3 mmol/L (100 to 129 mg/dL)
- Borderline high: 3.4 to 4.1 mmol/L (130 to 159 mg/dL)
- High: 4.1 mmol/L (160 mg/dL) or above
People with existing heart disease, diabetes, or a calculated 10-year cardiovascular risk above 7.5% are given lower LDL targets, often below 1.8 mmol/L (70 mg/dL). Your doctor will apply the right target for your personal risk level, not just the population average.
One raised LDL is not an automatic statin prescription
A single elevated LDL result does not mean medication is the next step. Your doctor will look at the full picture: your HDL, blood pressure, blood sugar, age, smoking history, and family history. Many people with borderline LDL bring it down with changes to diet and activity. A clinical assessment of your overall cardiovascular risk shapes the treatment decision, not one number in isolation.
HDL cholesterol: why higher is better
An HDL cholesterol level above 1.6 mmol/L (60 mg/dL) is considered cardio-protective by the NCEP ATP III, and counts as a negative risk factor, meaning it offsets one other risk factor in the cardiovascular risk calculation. HDL carries cholesterol away from artery walls and back to the liver for disposal, which is why it is called 'good' cholesterol.
- Low (risk factor): below 1.0 mmol/L (40 mg/dL) for men; below 1.3 mmol/L (50 mg/dL) for women
- Good: 1.3 to 1.5 mmol/L (50 to 59 mg/dL)
- Protective: 1.6 mmol/L (60 mg/dL) or above
Low HDL is an independent risk factor for heart disease. It often rises with regular aerobic exercise, weight loss, and stopping smoking. Very high HDL levels, above about 2.3 mmol/L (90 mg/dL), are common in people who exercise a great deal and are generally considered protective, though some genetic conditions that cause very high HDL do not always reduce cardiovascular risk.
Triglycerides: the sugar-and-fat connection
Triglycerides below 1.7 mmol/L (150 mg/dL) are normal; levels above 5.7 mmol/L (500 mg/dL) significantly raise the risk of acute pancreatitis and should be reviewed urgently by a clinician. Triglycerides are stored body fat that gets released into the bloodstream when you are not eating. They also rise sharply after a meal, which is why the test must be done fasting.
- Normal: below 1.7 mmol/L (below 150 mg/dL)
- Borderline high: 1.7 to 2.2 mmol/L (150 to 199 mg/dL)
- High: 2.3 to 5.6 mmol/L (200 to 499 mg/dL)
- Very high: 5.7 mmol/L (500 mg/dL) or above
Elevated triglycerides are strongly linked to a diet high in refined carbohydrates and added sugar, as well as to alcohol, physical inactivity, insulin resistance, and certain thyroid or kidney conditions. In urban Ghana, where a diet of refined carbohydrates, white rice, and sugary drinks is common alongside desk-bound work, high triglycerides often travel with elevated blood sugar and a raised waist measurement.

The total-to-HDL ratio: a quick risk check
A total cholesterol to HDL ratio below 4.0 generally indicates lower cardiovascular risk; many cardiologists aim for 3.5 or below. This ratio captures the balance between the cholesterol being deposited in arteries and the cholesterol being cleared away. Some labs calculate and print it automatically; if yours does not, divide your total cholesterol by your HDL.
For example: total cholesterol 5.0 mmol/L divided by HDL 1.4 mmol/L gives a ratio of 3.6, which is within the lower-risk range even though the total cholesterol is near the borderline. The ratio helps explain why two people with the same total cholesterol can have very different risks.
Why the test is done fasting
A 9 to 12 hour fast before a lipid profile is required because food temporarily raises triglycerides and can slightly change HDL, making the result harder to interpret. Total cholesterol is less affected by a recent meal, but because the complete panel is more useful than total cholesterol alone, the whole test is standardised to a fasted state.
Most people take the test early in the morning after an overnight fast, which makes the requirement easy to meet without discomfort. Water is fine to drink. If you take regular medications, ask your doctor whether to take them before or after the blood draw, as some drugs can affect lipid readings.
If you eat something by mistake before the test, reschedule rather than go ahead. A non-fasted triglyceride result can read twice as high as a fasted one and will not reflect your true baseline.
What this means in Ghana
Cardiovascular disease is the leading cause of non-communicable disease deaths in Ghana, according to WHO estimates. Several factors in the Ghanaian urban diet and lifestyle push lipids in the wrong direction: palm oil is high in saturated fat, refined carbohydrates raise triglycerides, and sedentary desk work reduces HDL-boosting physical activity.
Diabetes and prediabetes, which are rising rapidly in Ghana, compound the picture. High blood sugar raises triglycerides and lowers HDL through shared insulin-resistance mechanisms. This means that if your fasting glucose or HbA1c has ever been flagged, a lipid profile is particularly important to run alongside it, not separately.

What to do with your lipid profile result
- Read all four numbers, not just total cholesterol. LDL and HDL tell the more important story.
- Check the ratio. Divide your total cholesterol by your HDL. A result below 4.0 is the target.
- If LDL is borderline high (3.4 to 4.1 mmol/L), discuss dietary changes first: reduce saturated fat from processed foods and red meat, increase fibre, and aim for 30 minutes of brisk walking most days.
- If triglycerides are elevated, look at carbohydrate and alcohol intake first. These are the most diet-responsive of the four lipid markers.
- Get your blood sugar tested at the same time if you have not done so recently. High lipids and high blood sugar often have the same root cause.
- Retest on the schedule your clinician recommends: every 4 to 6 weeks if you have made a dietary change and want to see the effect, or every 1 to 3 years if results are normal and risk is low.
